Folded sheet, first verso and conjugate leaf blank. Drop-head title. Requiring Justices of the Peace to make a return on the enclosed form, establishing their qualifications and credentials in compliance with the directions of Viscount Duncannon.

Copy Notes Copy 1: Complete with blank proforma sheet to be used in the return, loosely inserted.
Copy 2: 37.4 cm. Wanting the blank proforma sheet used in Soane's return. Inscribed in ink at head Duplicate, and at foot in Soane's hand The Paper left at the Sessions House 5th. March 1835 / GB i.e. George Bailey, Soane's assistant. Previously folded as a letter inscribed in ink on blank final page Sir John Soane.
