Folded half-sheet, first verso and conjugate leaf blank. Drop-head title. Printed letter signed 'Geo: L. Tuthill', with the form Sir inscribed in ink. Simmons had announced his resignation as physician of St. Luke's Hospital for Lunatics, of which Soane was a governor.

Copy Notes Previously folded as a letter addressed in ink to J. Soane, Esqr. / Lincoln's-Inn-Fields, red wax seal.
