This drawing, SM 66/5/4, SM 66/5/3 and SM 66/5/5 show the same part of Wren's Hospital as SM 66/5/2 and SM 66/5/6, though they are dated later and relate to later alterations: slightly varied room allocations ('Secretary' is labelled on SM 66/5/4 of a room that was previously the Comptroller's) and some pencil additions including staircase alterations. The layout of rooms is similar in both sets of drawings.

As this drawing shows, the Physician had apartments on the first floor. The Board of Commissioners' minutes for February 1820 record Soane's estimates for repairs to these quarters and the Commissioners' orders that the Physician should be found accommodation elsewhere. The board also asked Soane 'to submit a Plan and Estimate for altering the Premises situated between the Apartments of the First Chaplain and the Comptroller, in such manner as to afford each of those Officers the accommodations solicited by them in their Memorial'. The drawing is evidently a survey for the proposed division of the Physician's apartments on the first floor, between the Chaplain and Comptroller, though it is unclear how the rooms were to be divided.
