A printed half-sheet (verso blank) containing a statement of instructions accompanied by two certificates on separate half-sheets: a printed proforma for the 'Petition' and a proforma for the certification having the docket title 'The Form Of The Certificates, Which, when properly signed and attested, are to be annexed to the Petition, and then left with the Secretary to be registered'. The watermark of the form in use as the petition is dated '1814'. The name of the secretary is given as 'John Webster'. Soane was one of the governors of St. Lukes.
